Connective tissue diseases (also termed connective tissue disorders, or collagen vascular diseases), are medical conditions that affect connective tissue. Connective tissues protect, support, and provide structure for the body's other tissues and structures. They hold the body's structures together. Connective tissues consist of two distinct proteins: elastin and collagen. Tendons, ligaments, skin, cartilage, bone, and blood vessels are all made of collagen. Skin and ligaments also contain elastin. These proteins and the surrounding tissues may suffer damage when the connective tissues become inflamed. The two main categories of connective tissue diseases are (1) a set of relatively rare genetic disorders affecting the primary structure of connective tissue, and (2) a variety of acquired diseases where the connective tissues are the site of multiple, more or less distinct immunological and inflammatory reactions. Diseases in which inflammation or weakness of collagen tends to occur are also referred to as collagen diseases. ==== Oral drugs ==== Gelatin is a common excipient in oral pharmaceuticals, both drug and vitamins. It was originally used in the shells of all capsules to make them easier to swallow. Now, a vegetarian-acceptable alternative to gelatin, hypromellose (hydroxypropyl methylcellulose, HPMC), is also used for hard capsules. It is less expensive than gelatin to produce. Modified starch has also been used. Softgels (soft capsules) remain mostly made of gelatin due to the flexibility needed. The first commercially-viable vegetarian alternative based on carrageenan-modified starch appeared in 2001. Modern ones also use alginate. The production process for vegetarian softgels remain more complicated than gelatin-based ones.

Diacetyldihydromorphine (also known as Paralaudin, dihydroheroin, acetylmorphinol) is a potent opiate derivative developed in Germany in 1928 which is rarely used in some countries for the treatment of severe pain such as that caused by terminal cancer, as another form of diacetylmorphine (also commonly known as Heroin). Diacetyldihydromorphine is fast-acting and longer-lasting than diamorphine, with a duration of action of around 4–7 hours. As an ester/analogue of dihydromorphine, diacetyldihydromorphine is presumably a Schedule I/Narcotic controlled substance in the United States but does not have its own ACSCN or annual production quota. It does appear in the German Betäubungsmittelgesetz and other European controlled-substances laws. Diacetyldihydromorphine is quickly metabolized by plasma esterase enzymes into dihydromorphine, in the same way that diamorphine is metabolized into morphine. Diacetyldihydromorphine is roughly equipotent to morphine, where as diamorphine (heroin) is 1.50–1.80 times the potency of morphine. It shares with other opioids the risk of overdose or (potentially life-threatening) respiratory depression. When strong narcotics are required, and morphine and diamorphine are not an option, it is more common to use better known drugs such as nicomorphine, hydromorphone, levorphanol, oxymorphone or fentanyl which doctors will be more familiar with, and which do not share the stigma associated with either heroin or morphine.

== History == Nitrogen mustards arose from the derivatization of sulphur mustard gas after military personnel exposed to it during World War I were observed to have decreased white blood cell counts. Since the sulphur mustard gas was too toxic to be used in humans, Gilman hypothesized that by reducing the electrophilicity of the agent, which made it highly chemically reactive towards electron-rich groups, then less toxic drugs could be obtained. To this end, he made analogues that were less electrophilic by exchanging the sulphur with a nitrogen, leading to the nitrogen mustards. With an acceptable therapeutic index in humans, nitrogen mustards were first introduced in the clinic in 1946. Aliphatic mustards were developed first, such as mechlorethamine hydrochloride (mustine hydrochloride), which is still used in the clinic today. In the 1950s, aromatic mustards like chlorambucil were introduced as less toxic alkylating agents than the aliphatic nitrogen mustards, proving to be less electrophilic and react with DNA more slowly. The Committee for Nuclear Information is frequently credited for its significant contribution to supporting the test ban, as is the ground-breaking research conducted by Reiss and the "Baby Tooth Survey". Public pressure and the frightening results of the CNI research led to a moratorium on above-ground nuclear weapons testing, followed by the Partial Test Ban Treaty, signed in 1963 by John F. Kennedy and Nikita Khrushchev. On the day that the treaty went into force, October 10, 1963, the Nobel Prize Committee awarded Pauling the Nobel Peace Prize for 1962.

== Chemistry == Pentobarbital is synthesized by methods analogous to that of amobarbital, the only difference being that the alkylation of α-ethylmalonic ester is carried out with 2-bromopentane in place of 1-bromo-3-methylbutane to give pentobarbital. Pentobarbital can occur as a free acid but is usually formulated as the sodium salt, pentobarbital sodium. The free acid is only slightly soluble in water and in ethanol while the sodium salt shows better solubility.
==== Comeback attempt and retirement (2006) ==== On January 23, 2006, the Astros indicated that they would file a claim on an insurance policy on Bagwell's health to collect approximately $15.6 million of the $17 million in salary owed to him for the 2006 season. Days earlier, orthopedic surgeon Dr. James Andrews had performed a physical examination on Bagwell and determined that he had become "completely disabled" and was unable to play baseball again. Because of the language of the policy, the Astros could not release him without losing their settlement, nor could he take the field. The decision effectively eliminated his chances of playing again in the Major Leagues. On March 28, Cigna rejected the claim, contending that because Bagwell had played during 2005 World Series he could not have become more disabled during the period of baseball inactivity characteristic of the offseason. Nonetheless, Bagwell still reported to spring training hoping he could contribute in some way during the upcoming 2006 campaign, and to test the injured shoulder. His mere presence in camp put the Astros' insurance claim into further question, creating an awkward situation between the player and the team. He appeared in 14 spring training games, batting .219 with one double. He never had to make any difficult throws that would place notable stress on his shoulder, as the other infielders shifted toward him. As expected, the Astros put him on the 15-day disabled list in late March with bone spurs in the shoulder.
