=== Tricyclic antidepressants === TCAs were the first medications that had dual mechanism of action. The mechanism of action of tricyclic secondary amine antidepressants is only partly understood. TCAs have dual inhibition effects on norepinephrine reuptake transporters and serotonin reuptake transporters. Increased norepinephrine and serotonin concentrations are obtained by inhibiting both of these transporter proteins. TCAs have substantially more affinity for norepinephrine reuptake proteins than the SSRIs. This is because of a formation of secondary amine TCA metabolites. In addition, the TCAs interact with adrenergic receptors. This interaction seems to be critical for increased availability of norepinephrine in or near the synaptic clefts. Actions of imipramine-like tricyclic antidepressants have complex, secondary adaptions to their initial and sustained actions as inhibitors of norepinephrine transport and variable blockade of serotonin transport. == Carbamino compounds in blood == The concentration of carbamate (HbCO2) was estimated in oxygenated and deoxygenated red blood cells of adult and fetal humans. The bicarbonate concentration in the red cells was calculated using the Donnan ratio for chloride and bicarbonate ions. Based on this figure, the carbamate concentration was determined by subtracting the bicarbonate concentration and dissolved CO2 from the total CO2 concentration. Deoxygenated fetal red cells contain more HbCO2 than deoxygenated adult red cells at a given pH value in the red cell. Upon oxygenation, HbCO2 decreased in both types of erythrocytes to values lower than in deoxygenated cells, at a constant pH. The fraction of 'oxylabile carbamate' (-ΔHbCO2/ΔHbO2) at a red cell pH of 7·2 and a PCO2 of 40 mm Hg is 0·117 in fetal and 0·081 in adult erythrocytes. == Adverse effects == Adverse effects, especially with higher doses, include dizziness, drowsiness, fatigue, diarrhea, unusual dreams, trouble sleeping, depression, and vision problems such as blurred vision or dry eyes. β-blockers, including metoprolol, reduce salivary flow via inhibition of the direct sympathetic innervation of the salivary glands. Metoprolol may also cause the hands and feet to feel cold. Due to the high penetration across the blood–brain barrier, lipophilic beta blockers such as propranolol and metoprolol are more likely than other less lipophilic beta blockers to cause sleep disturbances such as insomnia, vivid dreams and nightmares. ==== Ketogenesis and ketoacidosis ==== Ketogenesis is a condition in which the liver releases ketone bodies, i.e., acetoacetic acid, beta-hydroxybutyric acid, and acetone, into the blood. This occurs when blood glucose levels are moderately low such as during sleep, fasting, dieting, pregnancy, and the first 28 days after birth (i.e., the neonatal period); this form of ketogenesis is associated with modest elevations in the blood levels of the ketone bodies and, due to their increased release from adipose tissue, fatty acids. The circulating ketone bodies and fatty acids serve as nutrients to sustain the functioning of critical organs such as muscle, heart, kidney and brain when blood glucose levels are too low to do so. During serious stress conditions such as diabetic ketoacidosis and non-diabetic ketoacidosis due to excessive alcohol intake, medications, toxins, or starvation (see ketogenesis sections on each of these conditions), blood glucose levels are very low, blood ketone bodies and fatty acid levels are very high, and (due to the high blood levels of the ketone bodies and fatty acids) the blood is extremely acidic. == Adverse effects == Adverse effects include reactions at the injection site, such as a hot or painful feeling as well as general reactions such as nausea and vomiting.
